I think you’re moving in the right direction in assessing this, from the suggestion of a series of minor infringements to the more likely wilful neglect with financial motive or unchecked incompetence.
A lot of that could come down to personal interpretation of what is negligence or error, a lot of people have the opinion that the use of a flammable product was wrong right from the off so would immediately see that as a cost cutting and jeopardising lives but these sort of materials have been used successfully on countless other projects when installed in the correct way.
Certainly see plenty of poor design through many construction disciplines, along with just as much poor workmanship on site, that leads to the finished article not quite being what it could or should have been, it's not always down to cost or negligence though, sometimes there are genuine mistakes through lack of knowledge or attention, sometimes things just develop at later stages and some elements become fudged to an extent as it's too late to go back and start everything from scratch.
