Once you go through all the possible permutations then yeah.

Ireland and the Netherlands had identical, unsplittable records at Italia 90 for 2nd and 3rd - both went through but they had to draw lots for who finished higher. Ireland won so they played Romania next and the Dutch got West Germany.
