OK, the following happens, and in this order:
* Any clubs banned from UEFA competition cease to exist for these purposes.
* 3 places into the Champions League group stages from the top three clubs in the Premier League
* 1 place into the Champions League group stage for the winners of the previous season, unless they are one of the above.
* 1 place into the Champions League group stage for the winners of the Europa League, unless they are one of the above.
* 1 place into the Champions League group stage for the team finishing in fourth place, unless both the Champions League and Europa League winners are both English clubs, in which case this team goes into the Europa League group stage.
* 1 place into the Europa League group stage from the next placed club in the Premier League
* 1 place into the Europa League group stage for the winner of the FA Cup. If this team has already qualified, then the next position team in the Premier League qualifies.
* 1 place into the Europa League third qualifying round for the winner of the League Cup. If this team has already qualified, then the next position team in the Premier League qualifies.
* If the Europa League winner fills one of the Europa League spots available, then the "League Cup" place is lost and the remaining teams are allocated places in the group stage.
Basically, there is a maximum of 5 teams from any one country in the Champions League, and 4 in the Europa League if both winners don't qualify through the normal criteria.
As it stands, Man City don't count and have already won the League Cup. Therefore:
Positions 1-5 (minus City) go into the Champions League group stage
Position 6 gets to go into the Europa League group stage
FA Cup winners get into the Europa League group stage
Position 7 gets the League Cup space into the qualifying stages
